Abstract

This report summarises a national mapping and co-design process led by DHI, CFSD, and NHS Scotland, focused on harmonising heart failure diagnostics pathways. The project engaged thirteen health boards to develop a preferred digital model for referral, vetting, and diagnostics, aiming to streamline patient pathways and enable scalable, digitally enhanced decision-making.
