Smoking of tobacco being the major risk factor, the incidence of the disease is expected to increase by 33% in the next 20 years A Pharmaceutical Care Plan (PCP) is a systematic documentation of
patients’ pharmaceutical care issues, the desired outcomes and the required actions that need to be undertaken to fulfil these outcomes.
