A whole day exploratory workshop entitled “Potential of digital solutions for integration of health and social care”, (23rd Nov 2016) – arranged on request by the Aberdeen City Health and Social Care Partnership (ACHSCP). The Exploratory investigated the integration of services using digital technologies by focussing on three major themes: end-of-life-care, hospital at home and management of long-term conditions. The ACHSCP was looking to implement digital solutions in three care facilities across Aberdeen, and this event supported them to ascertain how to do this, and what types of solutions are available. A report with 8 recommendations for the ACHSCP was written. The most important ones of these were a) designing shared information and data governance protocols for use between the health and social care staff, and b) the implementation of a joint information and data sharing platform to support the integrated services and person-centred care.Period | 23 Nov 2016 |
